Microbial status of the unopened ophthalmic drops

Abstract Details

Purpose:

Our aim was to investigate microbial status of the un-opened ophthalmic drops that were used in Ophthalmology Departments.

Setting:

Anadolu University, Faculty of Science, Department of Microbiology

Methods:

Twenty five un-opened ophthalmic drops were plated on blood agar, chocolate agar, MRS agar, M17 agar, calcium-lactate agar, plate-count agar, Sabouraud-dextrose agar, malt agar, brain heart agar and MacConkey agar. Presumptive isolates were identified by morphological, and biochemical tests, and confirmed by the VITEK system. Biofilm formation capacities of the isolates were assessed with microtiter plate method. We determined the antibiotic resistance of the isolates by the Kirby-Bauer disc diffusion test. Molecular characterization of isolated bacteria from eye drops were performed using sequencing analysis of the 16S rRNA gene.

Results:

In 12 eye drops from 25 (7 antibiotics, 4 anti-glacoma drops, 4 anti-histaminic, 4 artificial tear eye drops, 4 midriatics, 2 topical anesthetics), 57 bacterial isolates were identified. The highest isolation rate was in artificial tears (100%) and the lowest was in antibiotics (0%). 25 out of 57 isolates were S. epidermidis (44%) and was followed by C.aurimucosum, B.subtilis, B.amyloliquefaciens, S. pasteuri (5 isolates for each). Forty isolates had moderate or strong biofilm production capacity. Eighty percent of bacteria were resistant to penicillin, and 42% were resistant to Moxifloxacin, Lemofloxacin and Ceftazidime. The lowest resistance rate was for Cloramphenicol (3.6%).

Conclusions:

We obtained isolates from approximately half of the un-opened bottles. 2/3 of those isolates had property of moderate or strong biofilm production. Antibiotic resistance rates were also high especially anti-bacterial eye drops. Those findings should be kept in mind when unexpected ocular infections are encountered.
