Predicting troublesome post FemtoLASIK effects

Abstract Details

Purpose:

To develop a multivariate mathematical model for prediction of vision quality impairment (VQI) and physical ocular discomfort (POD) after myopic femtoLASIK (laser-assisted in situ keratomileusis with femtosecond laser-assisted flap formation) with standard aspheric (SA) and wavefornt-guided (WFG) ablation protocol.

Setting:

Belarussian state medical academy of postgraduate education, Republican ophthalmic center, 10-th Minsk city hospital

Methods:

Data of 151 patients (302 eyes) with myopia and myopic astigmatism that undergone femtoLASIK procedure (101 – SA, 50 – WFG) was analyzed. Data included full ophthalmic anamnesis with complaints on VQI, POD, UCVA and BSCVA measurement, VSQoL (NEI-VFQ-25 questionary) and aberrometry before and 6 months after the operation. Prediction of vision disturbance and presence of POD was performed by the method of logistic regression based on preoperative data and multivariate analysis with backward variables selection. Prognostic nomogram was developed for the model use convenience. Nomogram precision was evaluated by concordance index (receiver operating characteristic (ROC) curve) and calibration plot.

Results:

Six parameters were included in the resulting model after mono- and multivariate analysis for VQI prediction, seven -- for POD prediction. Predictive accuracy of the model's amounted to 75,7% for VQI and 77,6% for POD, and the probability of abovementioned parameters could be represented by the nomogram in the range of 0,05% – 99,0%.

Conclusions:

The developed multivariate mathematical model's with graphical nomogram's can be convenient tools for predicting of troublesome post-femtoLASIK effects and appropriate ablation protocol selection and improve the communication between doctor and his patient on preoperative stage by predicting VQI and POD presence after myopic SA and WFG femtoLASIK.
