Management of irregular astigmatism after complicated small-incision lenticule extraction surgery

Abstract Details

Purpose:

To report the utility of CIRCLE software in managing irregular astigmatism following complicated SMILE surgery.

Setting:

Nethradhama Superspeciality eye hospital, Bangalore

Methods:

Two cases of irregular astigmatism after complicated SMILE were managed using the CIRCLE software of visumax laser .This software converts the cap into a flap, thus making access to the irregular interface possible. Case 1 was 26 year male, who presented 1 month after SMILE with reduced uncorrected distance visual acuity(UDVA) amd mixed astigmatism in both eyes. Clinical photography showed very rough interface with pitting in both eyes.Case2 presented 3 months post SMILE, with Corrected Distance Visual Acuity(CDVA) of 20/40 with acceptance of +1.5/ -2.00 D @70 in the left eye.ASOCT revealed localized irregularity and scarring,but no obvious lenticular tissue.

Results:

In both cases,CIRCLE software was used to convert the original cap into a flap to obtain access to the interface. In case 1, topo-guided treatment was performed on the irregular interface followed by washing of the interface with normal saline and reposition of the flap. In case 2, after lifting the flap, a small ledge of tissue was found in the interface,which was scraped,followed by which a 20 ยต topo smoothening was done. Both patients showed significant improvement in UDVA and CDVA on day 1 post repair surgery.

Conclusions:

CIRCLE software may be successfully used for treating irregular astigmatism apart from performing enhancement after SMILE.
