Abstract Details

Purpose:

Chopping a soft nucleus is not easy because it is too soft for a sharp chopper to provide sufficient counter force to crack. Femtosecond laser is useful to fragmentate such a nucleus, however the posterior plate will still remain undivided. Phaco prechopper is an ideal device to manage such cases. The purpose of this study is to develop a new prechopper for soft nucleus, which can be used easily and safely to attain the complete nuclear division prior to the phacoemulsification.

Setting:

The clinical test was performed at the operation theater of Akihabara Eye Clinic

Methods:

The prechopper has a cross action mechanism. The closed sharp angular blades are inserted directly into the nucleus. By grasping the handle, the blades will open to bisect the nucleus. The blade thickness was minimized for easier insertion and their opening was maximized through a small incision. The prechopper blade has a unique design with sharp angular and very round blunt edge which is used to ascertain the complete division. The maximum size of the blade width is 2.2mm which can be inserted through a small incision of MICS.

Results:

As the blade thickness was extremely thin, it was quite easy to insert into the nucleus without stressing the ciliary zonules. When the whole blade head was inserted into the nucleus, even a soft nucleus could be divided easily by squeezing the handle. The rounded side of the blade was safely used to attain complete separation of the posterior plate. The manipulation of the blade was much easier than the conventional combo prechopper because of the unique shape and small size of the blade. By prechopping the nucleus, following phacoemulsification could be performed quite easily and rapidly.

Conclusions:

The new prechopper is a useful device for prechopping a soft nucleus completely to facilitate the phacoemulsification.
