Abstract Details

Purpose:

To compare visual quality after posterior chamber phakic intraocular lens with a central hole implantation(ICL V4c) and femtosecond laser-assisted in situ keratomileusis(FS-LASIK) for moderate to high myopia.

Setting:

Peking University Third Hospital, Beijing Key Laboratory of Restoration of Damaged Ocular Nerve

Methods:

ICL V4c eyes and FS-LASIK eyes were well matched for age, gender, and mean level of preoperative spherical equivalent refraction. 1 month postoperatively, spherical equivalent refraction and best spectacle-corrected visual acuity were measured. Ocular higher-order aberrations were measured for 6mm pupils. Contrast sensitivity and glare contrast sensitivity function were measured in photopic and scotopic condition. We assessed the modulation transfer function cutoff frequency, objective scattering index, meanOSI, Strehl ratio and OQAS values at contrasts of 100%, 20%, 9%. We used the National Eye Institute Vision Function Questionnaire to investigate the effects on patients’ vision-related daily activities.

Results:

SE postoperatively was significantly greater in the FS-LASIK group than in the ICL V4c group. Total HOAs, CA, SA, Fourth-order aberration, Sixth-order aberration were greater in the FS-LASIK group than in the ICL V4c group(P=0.042, P<0.001, P<0.001, P<0.001, P=0.006). There were significant differences between two groups in photopic CS with no glare(3c/d,P<0.001, 6c/d,P<0.001,12c/d,P<0.001,18c/d, P<0.001) postoperative. The two groups showed no significantly CS in scotopic with and without glare condition. We found no significant differences in the MTFcutoff frequency, OSI, meanOSI, SR, OV100%, OV20%, OV9% with two groups. No differences with vision-related daily activities.

Conclusions:

ICL V4c implantation have decreasing in spherical equivalent than FS- LASIK group. ICL V4c implantation have significantly fewer ocular HOAs than FS-LASIK. Particularly Coma and Spherical aberrations play an important part in the BSCVA. The CS of ICL eyes were better than FS-LASIK eyes in photopic with no glare condition in all 3、6、12、18c/d. The OQAS value for example OSI, meanOSI, MTFcutoff frequency, Strehl ratio and OQAS values(OVs) at contrasts of 100%, 20% and 9% after surgery provide excellent outcomes between the two group. Questionnaire showed that both groups were satisfied with their daily life.
