Long-term survival analysis of iris-fixated phakic intraocular lenses

Abstract Details

Purpose:

To determine the rates of explantation and survival time of several different types of phakic intraocular lenses (pIOLs), and to assess risk factors for pIOL explantation.

Setting:

Academic Center for Refractive Surgery, University Eye Clinic Maastricht, Maastricht University Medical Center, Maastricht, The The Netherlands.

Methods:

Survival analyses were performed using data from patients implanted with six types of iris-fixated pIOLs (Artiflex, Artiflex Toric, Artisan Toric, Artisan Hyperopia, Artisan Myopia [Ophtec, Groningen, the The Netherlands]), from 1998 until November 2016. Thousand-thirty-nine eyes with a mean preoperative age of 40.1±11.0 were included. Multiple measurements were performed preoperatively, and in succession 1 day, 1 week, 1, 3, 6 and 12 months postoperatively, followed by annual visits after completing the first year of follow-up. Kaplan Meier and multivariate Cox regression analyses were performed to report survival time and asses risk factors for pIOL explantation.

Results:

The overall rate of explantation in this study was 12% (n = 120). We report an estimated median survival (i.e. time until 50% of pIOLs is explanted) of 183 months (15.3 years). Patients with a higher preoperative age (HR 1.07/year, 95% CI=1.04–1.09, p < 0.001), greater axial length (HR 1.10/mm, 95% CI = 1.02–1.18, p = 0.009), or smaller anterior chamber depth (HR 4.47/mm, 95% CI=2.43–8.22, p < 0.001) are likely to have a shorter survival. Analyses describing risk factors for explantation due to cataract or endothelial cell loss, and differences between lens types, will be presented.

Conclusions:

It is estimated that it will take 15.3 years until 50% of all pIOLs are explanted. Higher age at implant, greater axial length and smaller anterior chamber depth significantly increase the chance of a shorter survival in eyes implanted with iris-fixated pIOLs.
