Abstract Details

Purpose:

Precise refractive outcomes are essential for the success of premium intraocular lenses (IOL’s). Multicomponent IOL’s (MCIOL) allow refinements after primary surgery by enabling surgical exchange of one of its refractive components. We demonstrate here the relative surgical simplicity of adopting this technology and present the early clinical outcomes of refractive enhancement procedures among eyes that underwent cataract surgery with MCIOL implantation.

Peregrine Eye and Laser Institute, Makati, Philippines

Methods:

Experimental design. After cortical removal, a pre-assembled MCIOL was injected into the capsular bag as a single injection step. Three months after this primary surgery, eyes with manifest refraction spherical equivalent (MRSE) greater than 0.75 diopters underwent an enhancement procedure, as follows: the front lens was separated from the base lens and removed through the original incision using IOL forceps; a new front lens with a different refractive power, based on 3-month MRSE, was injected into the anterior chamber and attached to the base lens. Main outcome measures: post-enhancement uncorrected distance visual acuity (UDVA) in logMAR and change in MRSE.

Results:

Of 34 eyes that received the MCIOL, 16 underwent the enhancement procedure. The mean (standard deviation) preoperative UDVA of 0.2 (0.15) logMAR improved to 0.0 (0.07) logMAR after enhancement (P=0.008). The mean pre-enhancement MRSE of +1.39 (0.92) diopters significantly decreased to +0.09 (0.40) diopters after enhancement (P=0.0002). After enhancement, all eyes had UDVA of 0.1 logMAR or better. The enhancement procedures were completed within 5-minutes or less. None of the eyes developed significant adverse events.

Conclusions:

MCIOL provides a safe, consistent and efficient method of improving refractive and visual outcomes after cataract surgery. The enhancement procedure is fast, easy to learn and enables surgeons to optimize refractive outcomes.
