Treatment of myopia and astigmatism with topography-guided corneal ablation in primary eyes: preliminary results with T-CAT Contoura profile

Purpose:
To report the visual and refractive outcomes of customized topography-guided corneal ablation (T-CAT - Contoura Vision; WaveLight Alcon Laboratories) for myopia and astigmatism treatment in primary eyes.
Setting:
Ophthalmology Department – Hospital CUF Porto, Instituto CUF Porto; Portugal
Methods:
Prospective study, patients with a spherical equivalent of -6,0D or below, submitted to bilateral femtosecond laser assisted LASIK between January 2017 and December 2017 with a minimum follow-up of 3 months. A topography guided primary ablation profile (T-CAT Contoura Vision) was applied. Parameters evaluated: uncorrected and corrected visual acuity, subjective refraction. Corneal topography was obtained from the Allegro Oculyzer (Alcon Laboratories, Inc.); a 110 m thickness flap was performed with femtosecond laser WaveLight FS-200 (Alcon Laboratories, Inc.); ablation was performed with Allegretto Wave Eye-Q Excimer Laser (Alcon Laboratories, Inc.).
Results:
Study included 50 eyes of 25 patients, mean age 34,0 ± 6,15. The T-CAT Contoura profile effectively reduced the spherical equivalent from -2,75 ± 1,87D to +0,00 ± 0,18 D at 3 months; 100 % of eyes had a SE within ± 0,50 D. 32% of eyes achieved UCVA of 20/12.5 and 64% of 20/16; a postoperative UCVA same or better then preoperative BCVA was obtained in 91% of treated eyes; no eyes lost lines of BCVA and 55% gained 2 or more lines of BCVA. No enhancement procedure was performed; no intraoperative or postoperative flap interface complication was reported.
Conclusions:
Contoura T-CAT topography-guided ablation profile is a safe and effective treatment for myopia and astigmatism in primary eyes. The superior visual results obtained confirm the importance of customizing the corneal ablation to obtain better outcomes; the possibility to address the high order aberrations observed in the cornea of normal eyes enables significantly better visual outcomes when compared to any conventional ablation profile reported for corneal laser surgery in myopia and astigmatism.
