Purpose:
To describe a three stage flap replacement protocol, and report the incidence of microfolds after LASIK surgery.
Setting:
London Vision Clinic, London, UK
Methods:
This was a retrospective review of 14,374 eyes that underwent LASIK at London Vision Clinic, London, UK since 2010. Inclusion criteria were to have undergone LASIK with the flap created by the VisuMax femtosecond laser (Carl Zeiss Meditec). The first step in flap management occurs when repositioning the flap after completing the ablation. Immediately after treatment the patient is then moved to a slit-lamp where additional microadjustments can be made to the flap. Finally, further fine-tuning can be performed with a microspear sponge at the slit-lamp during the early postoperative visits.
Results:
There were 983 eyes (6.84%) that needed additional adjustment at the 1-day visit. Three months after surgery there was trace microfolds in 158 eyes (1.10%), grade 1 microfolds in 26 eyes (0.184%), and grade 2 microfolds in 2 eyes (0.016%) of eyes. A flap lift for visually significant microfolds was required for 1 eye (0.007%). The flap lift was performed at the 1 day visit in this patient to address a microfold that could not be managed at the slit-lamp.
Conclusions:
The three stage protocol for flap positioning and management resulted in a low number of eyes with clinically visible microfolds and no visually significant microfolds in this 14,374 eye population.
