Mixed astigmatism one-step correction by LASIK

Abstract Details

Purpose:

To evaluate the safety, efficacy and predictability, the visual refractive and aberrometric outcomes of Lasik one-step correction of mixed astigmatism with the new model of Technolas Teneo 317 software, to find the best strategy of correction.

Setting:

Tbilisi State Medical University, Eye Clinic "Akhali Mzera" Ltd, Tbilisi, Georgia

Methods:

Prospective part of the study included eyes with mixed astigmatism operated on the new model of Technolas Teneo 317. The results were compared to retrospectively analyzed cases of mixed astigmatism two-step correction on Technolas 217z100 . First group included 34 eyes of 26 patients with mixed astigmatism over 2.0 diopters (D). The second group included randomly assigned 50 eyes of 34 patients with mixed astigmatism operated in two steps with 1.5 – 2.0 months interval of time. All cases underwent LASIK surgery using new model of Technolas Teneo 317 for the I group and Technolas 217z100 for the II group.

Results:

A significant reduction of refractive sphere and cylinder was observed 1 months postoperatively (P = .001), with an associated improvement of uncorrected distance visual acuity (P = .001). Visual, refractive, corneal topographic, and aberrometric outcomes were evaluated during a follow-up period (at least 3 months). Refractive astigmatic changes were analyzed. Postoperative UCVA, BCVA, refractions (subjective and cycloplegic) showed no significant difference between groups. After surgery, UCVA was 20/30 or better in 75.50% and 53.3% in group I and II respectively. No lines of BCVA were lost. No postoperative complications were found in any group.

Conclusions:

LASIK with the new model of Technolas Teneo 317 software for mixed astigmatism correction is an effective, predictable, safe and tissue-friendly procedure with efficient treatment times and superior visual results. This technique allows us to avoid two-step surgery and nearly double financial expenses.
