Abstract Details

Purpose:

To update our study comparative models of concentration abatement in the anterior chamber to include all three most common prophylactic intracamerally administered antibiotics to better understand their relative durations of antibacterial efficacy.

Setting:

York Finch Eye Associates, and the University of Toronto

Methods:

Mathematical models of aqueous turnover were updated and applied to intracameral antibiotic dosing and bacterial resistance to yield comparisons of the three most commonly used agents.

Results:

Graphs of antibiotic concentration abatement were created to illustrate what happens under the most likely assumptions. The goal was to discover how long the concentrations of each antibiotic exceeds the bactericidal MIC90 of the most resistant bacteria that are usually encountered in cataract surgery. ICM retains antibacterial activity for 40 hours post injection, which is almost twice as long as IVC or ICV. The results reveal the relative efficacy preventing postoperative endophthalmitis and present a model that can be used to ascertain sensitivities of local pathogens.

Conclusions:

This study of models of intracameral antibiotic concentration abatement can enhance our understanding of the effects of our current protocols and suggest better practices, and can be used to ascertain the efficacy of local protocols versus local pathogens.
