Predictors of subjective satisfaction after cataract surgery

Abstract Details

Purpose:

The aim of this study was to study the link between objective parameters and subjective satisfaction after cataract surgery. Subjective outcome was measured using the Danish Catquest questionnaire, which recently passed Rasch validation.

Setting:

Friklinikken, Silkeborg, Denmark. Aalborg University Hospital, Denmark

Methods:

In a prospective study, 228 cataract patients eligible for cataract surgery were recruited. Preoperative (preOP) investigations included: the Catquest questionnaire, best-corrected visual acuity (BCVA), intraocular pressure, biometry, slit-lamp investigation, age, gender and notation of comorbidities. Three months postoperatively (postOP), patients filled out the Catquest again. The association between ΔCatquest-7SF (preOP Catquest-7SF – PostOP Catquest-7SF), BCVA (logMAR) in the best and worse eye, corneal astigmatism ≤ 1 diopter (D) or > 1 D in the best and worse eye, presence of comorbidity in the best and worst eye, preOP Catquest, gender and age was assessed.

Results:

Four variables were found to contribute to the model. After Bonferroni correction however, only preOP Catquest exhibited a significant correlation with (p<0.000) with a positive coefficient of 0.8 (0.6 – 1.0), which indicates that more difficulty experienced before surgery correlated to an increase in subjective improvement. Age and gender did not affect findings. Based on these results, a prediction interval of 95 % was made (figure 1). This showed, that if patients had a CatquestpreOP value of 0.5 or above there was a 95 % likelihood of seeing an increase in subjective outcome.

Conclusions:

Subjective satisfaction after cataract surgery was significantly correlated to preOP Catquest levels but not to any of the objective parameters included in this study. Measurement of Catquest-7SF before cataract surgery may help selection of cataract patients in order to ensure good subjective outcome.
