Purpose:
To compare the long-term efficacy and safety visual outcomes of Refined single-step TransPRK with those of Conventional single-step TransPRK in correction of mild-moderate myopia.
Setting:
Bina eye hospital, Tehran
Methods:
In this prospective comparative interventional case-series, patients with myopia (<5 diopters) with or without astigmatism were enrolled. The same expert surgeon (SAM) performed Conventional (group1, 88 eyes) and Refined (group2, 41 eyes) methods of single-step TransPRK using SCHWIND AMARIS 500 laser. Two methods mainly differed in nomograms used for determination of target refraction and optical zone, laser centration approach, and post-operative medical regimen. Mean age of the total patients was 27.88±5.05 and 95 eyes (73.6%) belonged to female individuals. Preoperative and 18-month postoperative visual acuity, refraction, predictability rates, safety and efficacy indices, and contrast sensitivity were compared.
Results:
Mean preoperative spherical equivalent(SE) and astigmatism of total group were -3.40±1.26 and -1.28±1.11D, respectively. Age, gender distribution and preoperative SE, astigmatism, visual acuity and contrast sensitivity were comparable between two groups. Compared to Conventional group, Refined group achieved better postoperative logMAR-UDVA (-0.14±0.06 vs -0.08±0.09, p=0.001) and -CDVA (-0.16±0.04 vs -0.10±0.04, p=0.001). Safety and efficacy indices were significantly better in the Refined group (1.80±1.06 vs1.19±0.97, p=0.006; and 1.56±1.19 vs 1.01±1.3,1 p=0.045). Furthermore, Refined group achieved better postoperative contrast sensitivity results. Correction of spherical refraction and astigmatism were comparable between two groups.
Conclusions:
Correction of mild to moderate myopia with or without astigmatism by Refined single-step TransPRK resulted in better visual acuity as well as safety and efficacy indices compared to Conventional method. Significant difference was not detected in indices of spherical refraction and astigmatism correction.
