Purpose:
late in-the-bag intraocular lens(IOL) dislocation is a serious complication of modern cataract surgery which may compromise an early (immediately after cataract operation) excellent postoperative result. Various risk factors have been appointed so far in the literature but the several years’ interval from cataract surgery for this rare complication to appear makes difficult to stress out the preoperative (before cataract operation) characteristics of these eyes.
Setting:
General Hospital of Lamia, Lamia, Greece
Methods:
retrospective case-control study of a series of 12 patients(10 men,78-85 years old and 2 women,79 and 84 years old) who presented with in-the-bag intraocular lens dislocation several years(median 9,range 7 to 10) after having successful phacoemulsification in General Hospital of Lamia. The cases were matched with 48(1:4 ratio) consecutive patients who were operated at the same period in General Hospital of Lamia and for whom ten years’ follow up was available. Retrospective chart review was undertaken for all patients in the study. Preoperative(before cataract operation) characteristics as age, sex, previous ocular history, pseudoexfoliation syndrome, phacodonesis and glaucoma were recorded.
Results:
the following preoperative characteristics were revealed as statistically significant: (1) pseudoexfoliation was present in all cases (100%) while only in 40% of the controls. (2)Phacodonesis was present in 4 cases while in none of the controls (3) male sex differed from 10/12 in cases to 20/48 of the controls The study depicted significant correlation (Spearman’s rho, p<0.005) between late in-the-bag IOL dislocation and pseudoexfoliation (0.6449), phacodonesis(0.4588) and (female sex(-0.3333).
Conclusions:
pseudoexfoliation at the time of cataract operation was the most significant preoperative risk factor of late in-the-bag intraocular lens dislocation. Appropriate patient consultation and phacoemulsification technique adjustments are necessary in these cases.
