The rate of LASIK 'fine tuning' in patients post multifocal intraocular lens implantation

Purpose:
To evaluate the incidence of LASIK treatment to correct residual refraction after multifocal intraocular lens (MIOL) implantation.
Setting:
Tertiary referral center, Institute for Refractive and Ophthalmic Surgery (IROC)
Zurich, Switzerland.
Methods:
A retrospective study.
One hundred sixty-seven eyes of eighty-five consecutive patients who underwent a multifocal intraocular lens (Micro F, PhysIOL) implantation from 1.1.2015 till 31.5.2017 were evaluated. The number of eyes, which needed a LASIK treatment, was assessed. Criteria for retreatment were that patients were unhappy with some aspects of their vision (either blurred distant and/or near vision or visual symptoms) that was related to their residual refractive error. The patients have a follow-up of at least 6 months after implantation.
Results:
167 eyes were enrolled. 50 women and 35 men were included. The mean age was 62.94±9.88(min-max 39-83). Pre-operative 25(29%) patients were myopic and 60(71%) were hyperopic.
57(34%) eyes needed a LASIK enhancement. 44(77%) of those eyes had a spherical equivalent refraction within ±0.50 diopters, 13(23%) had a spherical equivalent refraction greater than ±0.50 diopters. 10(18%) eyes had an astigmatism >- 0.75.
After LASIK enhancement 54(95%) eyes had an unaided visual acuity better than 0.8sc.
There was no significant difference in age, gender, pre MIOL implantation refraction and dominant eye between LASIK treated and untreated eyes.
Conclusions:
Every third eye needed a LASIK enhancement treatment. This was not due to inadequate result of the MIOL implantation as 77% of the LASIK treated eyes, had a spherical equivalent refraction within ±0.50 diopters. The high visual quality expectations of patients, purchasing high end lens, might explain the high proportion of LASIK enhancement.
