Purpose:
Vitrectomy-associated changes in ocular surface (OS) are not yet fully understood, although a number of studies have reported OS changes after cataract surgery. The purpose of this study was to evaluate OS changes before and after vitrectomy in eyes with posterior segment diseases and to investigate their association with surgical procedures.

Osaka University Hospital, Osaka, Japan
Methods:
Thirty-five eyes of 32 consecutive patients (16 women, 16 men; average age 66.6 ± 11.1 years) were enrolled in this prospective, noncomparative case series study, from among patients who underwent primary 25-gauge vitrectomy. Patients were excluded for regular use of topical eye drops, contact lens wear, and/or any history of ocular surgery within the preceding 6 months. Tear break-up time, corneal and conjunctival fluorescein staining score (FSS), and tear meniscus height (TMH) were evaluated before, 1 week after, and 1 month after vitrectomy. Multiple linear regression analysis was performed to identify significant contributors to changes in OS parameters.
Results:
Conjunctival FSS and TMH were significantly increased at 1 week after vitrectomy, compared with preoperative measurements; they were significantly decreased at 1 month after vitrectomy, compared with 1 week after vitrectomy. Multiple linear regression analysis identified surgical time as a significant contributor to increased conjunctival FSS (P = 0.011) and combined cataract surgery as a significant contributor to decreased conjunctival FSS (P = 0.033). Sclerotomy site suturing was identified as a significant contributor to increased TMH (P = 0.025).
Conclusions:
This study showed associations between temporal postoperative OS changes and specific surgical procedures of 25-gauge vitrectomy. Caution should be exercised to minimize the effect of surgical procedures on the OS during vitrectomy.
