Dry eye and quantitative/qualitative changes of meibomian glands in pemphigus patients

Abstract Details

Purpose:

To evaluate the dry eye and quantitative/qualitative changes of Meibomian Glands (MGs) with meibography in pemphigus patients.

Setting:

Ege University Department of Ophthalmology

Methods:

Twenty left eyes of 20 inactive pemphigus patients (Group 1) and 20 left eyes of 20 age and sex matched healthy volunteers (Group 2) were enrolled. Dry eye was evaluated with ocular surface staining (Oxford scale scoring), tear film break-up time(T-BUT), Schirmer-1 test, and Ocular Surface Disease Index(OSDI) score assessment. Morphological changes and drop-out ratio of MGs were evaluated by upper and lower eyelid meibography using infrared captures of a biomicroscope(Topcon,SL-D701,IJssel,The The Netherlands). Partial or complete loss of the MGs was scored for each eyelid from grade 0(no loss) through grade 3(lost area was >2/3 of the total Meibomian gland area).

Results:

The mean ages of Group 1 and Group 2 were 51.8±11.1 (range, 32-73) and 50.4±9.6 years (range, 32-70), respectively (P=0.672). Among dry eye tests mean Schirmer-1 and T-BUT values were lower; mean Oxford and OSDI scores were higher in Group 1 and the difference was statistically significant (P=0.01, P<0.001, P=0.001, P<0.001 respectively). On the other hand, upper meiboscore, lower meiboscore and total (upper+lower) meiboscore were higher in Group 1 and the difference was also statistically significant (P=0.005, P=0.018, P=0.002 respectively). In pemphigus patients the morphological changes were detected in 16 eyes (80%).

Conclusions:

Pemphigus, mainly affects various mucosal surfaces of the body including conjunctiva. There are significant differences in dry eye tests and MG morphology in patients with pemphigus when compared with healthy individuals. Even in eyelids with minimum MG drop-out, morphological changes in MGs are severe. For this reason, pemphigus patients should be suspected and monitored for dry eye and MG dysfunction and promptly start treatment whenever needed in order to prevent serious consequences of dry eye.
