Purpose:
Eyes with myopic astigmatism may be eligible for full correction with Small
Incision Lenticule Extraction (SMILE). However, the lack of an automatic compensation of cyclotorsional eye movements is perceived as a major weakness of SMILE. At present, SMILE is only FDA-approved for use in myopic eyes with astigmatism of -0.5D or less. We report outcomes of SMILE in myopic eyes with astigmatism of 1 D or higher.
Setting:
Center for Refractive Surgery, Eye Department at St. Francis Hospital Muenster, Germany
Methods:
Observational consecutive case series. Inclusion criteria: Myopia -0.5 D or
more, astigmatism 1.0 D or more, and corrected distance visual acuity (CDVA) of 1.0
(decimal scale) or better.
Results:
In total 219 eyes were included. Preoperatively, mean attempted spherical equivalent correction was -5.4 ± 2.36D (range: -1.25 to -11.38D) and mean cylinder was -1.65 ± 0.77D (range: -1.0 to -4.75D). At 3 months postoperatively, mean spherical equivalent refraction was -0.06 ± 0.38D (range: -1.0 to +1.0D) and mean cylinder was -0.33 ± 0.28D (range: 0 to -1.5D). Mean UDVA was 1.11. Mean efficacy index was 0.85. Safety index was 0.99. UDVA same or better than CDVA: 66%. Angle of Error was ±5° in 69% and ±15° in 95%. Astigmatism was ≤ 0.5D in 88% and ≤ 1.0D in 100%.
Conclusions:
SMILE for the correction of myopic astigmatism was found to be safe and effective with outcomes equivalent to those previously reported for LASIK using modern tracking systems. Results may be improved in the future with eye-tracker guided compensation of cyclotorsional eye-movements.
