Orbital neoplastic lesions: review of three years of clinical practice

Abstract Details

Purpose:

To review surgically treated orbital neoplastic lesions at the Ophthalmology Department of a tertiary center.

Setting:

Department of Ophthalmology, Sao Joao Hospital, Porto, Portugal.

Methods:

All patients who underwent orbital surgery at the Department of Ophthalmology of Sao Joao Hospital between September 1st 2014 and August 31st 2017 were retrospectively reviewed. Those with orbital neoplastic lesions, confirmed by histopathological examination, were selected. Intraocular, eyelid and conjunctival neoplastic lesions with orbital involvement were excluded.

Results:

During the three-year period, eleven patients underwent surgery due to orbital neoplastic lesions. Eight (73%) were malignant and three (27%) benign. The most common neoplasm was lymphoma (45%). Age at surgery was, on average, 61 years, and varied between 17 and 89. Seven (64%) lesions involved only the extraconal compartment, two (18%) only the intraconal compartment, and the remaining two both orbital compartments and the pre-septal region. The most frequent complaints were orbital pain and eyelid edema. Purely intraconal lesions were associated with reduced vision. Generally (82%), the selected surgical approach was upper lid crease incision.

Conclusions:

Lymphoma was the most common surgically treated, orbital neoplastic lesion. Orbital pain and eyelid edema were the most frequent complaints. Anterior orbitotomy through an upper lid crease incision allowed a safe and simple approach to most lesions, with excellent cosmetic results.
