Abstract Details

Purpose:

To evaluate longterm surgical outcomes of in-the-bag single piece hydrophobic acrylic intraocular lens (IOL) implantation following pediatric cataract surgery.

Setting:

Iladevi Cataract & IOL Research Centre, Raghudeep Eye Hospital, Ahmedabad, India

Methods:

Retrospective study in 51 eyes(35 children) upto 15 years of age undergoing cataract surgery with in-the-bag single piece hydrophobic acrylic IOL implantation for non-traumatic cataracts. Those with atleast 6 years postoperative followup were included. All eyes had manual anterior and posterior capsulorhexis, anterior vitrectomy and in-the-bag single piece hydrophobic acrylic IOL implantation. Outcome measures evaluated at last followup were:Best corrected visual acuity(BCVA),incidence of glaucoma, retinal detachment, visual axis obscuration(VAO) and secondary intervention. IOL centration/dislocation were documented

Results:

Mean (+standard deviation (SD)) age at surgery was 47+44 months(range 3.5-112 months). Mean follow-up was 118 months (range 76-276 months). During the entire followup, 70% eyes developed VAO requiring surgical/laser intervention at a mean of 38 months from surgery. Glaucoma was seen in only 2 eyes (4%), both of which were well controlled with medications. BCVA improved in all eyes, and mean BCVA at final followup was:0.33+0.35 LogMAR. All eyes had well centered IOLs, and no IOL dislocation or retinal detachment occurred until final followup

Conclusions:

Longterm outcomes following cataract surgery with single piece hydrophobic acrylic IOL implantation in the bag are favourable with an acceptable rate of complications. However, VAO incidence is high in the longterm and continues to occur even after several years of followup.
