Abstract Details

Purpose:

To report the use of a PMMA monofocal IOL with prosthetic iris, the Aniridia Lens II (ANII; Ophtec; the The Netherlands) in five patients with photophobia and glare due to partial or total aniridia and cataract or aphakia.

Methods:

A single-center retrospective case series of 5 eyes of 5 patients implanted with a prosthetic iris IOL. Indication for implantation was photophobia and glare secondary to partial or total aniridia and either cataract or aphakia. The primary diagnosis was recurrent uveitis (n=1) or trauma (n=4). All patients implanted between September 2012 to February 2018 were included. Data were obtained from the electronic patient records (Epic). Primary outcome measures were visual acuity and refraction. Secondary outcome measures were complications.

Results:

After implantation of the ANII al patient were satisfied with the reduction of photophobia and glare. The visual outcome was good, e.g. 4 patients improved their vision up to 1,25 (decimal) and one patient maintained light perception secondary to a corneal scar. Postoperative follow-up time was 11.4 months SD 12.7. Long-term complications occurred in 2 patients and included retinal detachment (n=1), posterior capsular opacification (n=1) and extrusion of scleral imbrication element (n=1). After treatment, the complications did not influence the visual outcome or improvement of symptoms experienced by patients. Four patients were referred back to a secondary eye care center.

Conclusions:

The ANII is a valuable device in management of patients with glare and photophobia due to iris translucency after trauma or uveitis and concurrent aphakia or cataract. In our limited experience the prosthetic iris IOL allows for visual rehabilitation in the range of indications for this IOL: partial or total aniridia with cataract or aphakia.
