Abstract Details
Purpose:
To compare the visual outcomes following SMILE and FS-LASIK for correction of high myopic astigmatism (greater than or equal to -2.0D cylinder)
Setting:
The Eye Foundation, Coimbatore- A tertiary eye care hospital in South India
Methods:
A retrospective, interventional case series comparing the results of SMILE and FS-LASIK for myopic astigmatism was conducted between June 2014 to December 2016. Patients suitable for refractive correction with myopic astigmatism > 2.5D were included. SMILE was performed using Visumax (Carl Zeiss Meditec) platform without manual cyclotorsion compensation or the use of an additional nomogram. FS-LASIK excimer ablation was done using the EX 500(Wavelight, Allegretto). Visual outcomes were evaluated at 6 weeks and 3 months’ follow-up visit. The data was calculated using Alpins criteria.
Results:
The study included 100 eyes each with a mean preoperative cylindrical error of -2.71D and -2.86D in the SMILE and FS-LASIK group (P=0.18). 74% of eyes in SMILE group and 79% in FS-LASIK group had UDVA of 20/20 or better.5 eyes in SMILE gained 1 or more lines as opposed to 15 eyes in FS-LASIK.The difference vector between the 2 groups were not statistically significant [0.11+/-0.33 in SMILE vs 0.06 +/- 0.23 for FS-LASIK(p=0.17)]. Index of Success was similar between the two groups. 90% in SMILE and 94% eyes with FS-LASIK group achieved refractive outcomes within 0.5D cylinder.
Conclusions:
Our study showed mild under correction of astigmatism in both group. The target astigmatism correction achieved in SMILE without manual cyclotorsion compensation was similar to the results obtained in the FS-LASIK group
