Purpose:
To study the impact of cornea guttata on postoperative visual acuity and patients' self-assessed visual function after cataract surgery.
Setting:
Forty-nine Swedish cataract surgical units. Department of Clinical Sciences/Ophthalmology, Umea University.
Methods:
33741 patients from the Swedish National Cataract register were included with one eye each in this cross-sectional retrospective study. They all underwent cataract surgery in the month of March 2010-2017. The best corrected visual acuity (BCVA) 1-3 months and self-assessed visual function (Catquest-9SF) 3 months after the surgery was used as outcome measures. Logistic proportional odds regression model was applied because of non-normal distribution. The postoperative BCVA and Catquest index was cut into intervals in steps of 0.05 logMAR units and 0.5 logit units. Adjustments were made for age, sex, ocular comorbidities, time to follow-up and preoperative visual function.
Results:
The presence of cornea guttata was significantly associated with an inferior result considering both postoperative BCVA and Catquest, with an adjusted odds ratio of 1.97 (95% CI: 1.73-2.23, p < 0.001) of being in a higher BCVA category and 1.21 (95% CI: 1.08-1.36, p < 0.001) of having a higher index on Catquest. The median postoperative BCVA with guttata was 0.1 logMAR (IQR 0, 0.22) and without guttata 0 logMAR (IQR 0, 0.15), p < 0,001. The median postoperative Catquest index with guttata was -3.16 (IQR -5.08, -1.29) and without guttata -3.35 (IQR -5.13, -1.76), p < 0,001.
Conclusions:
Cataract surgery greatly improves the visual acuity and self-assessed visual function for cataract patients both with and without corneal guttata. Still, cornea guttata has a negative influence on the BCVA even 3 months after the surgery and the self-assessed visual function after 3 months.
