Purpose:
To describe the IOL prediction accuracy and refractive outcomes following implantation of a trifocal intraocular lens (IOL) using intraoperative wavefront aberrometry (IWA).
Setting:
Private practice - Unidade de Oftalmologia de Coimbra (UOC), IDEALMED, Coimbra, Portugal.
Methods:
Prospective study evaluating eyes undergoing cataract surgery with a trifocal IOL (PanOptix, Alcon, non-toric TFNT00 and toric TFNTx), aided by IWA (ORA, Alcon), which allowed intraoperative calculation of IOLs spherical and cylindrical power. The IOL power (surgeon best-choice, SBC) was also estimated preoperatively using optical biometry and, for toric IOLs, using the Alcon online tool including the Barrett calculator. Accuracy outcomes were the median absolute prediction errors (MedAE) for IWA and SBC and the proportion of eyes within 0.50 and 1.00 diopters (D) of this prediction. Refractive outcomes included the post-operative spherical equivalent (SE) and the refractive cylinder.
Results:
We included 150 eyes in this study. Regarding accuracy of IOL selection, IWA significantly outperformed the SBC (MedAE 0.18D vs 0.22D, p<0.003) and prediction accuracy was very high (within 0.5D 87.3%, within 1.0D 99.3%). IWA yielded a different IOL spherical power in 57.3% of cases. Mean post-operative refractive SE was -0.05±0.30D, with 94.7% of patients within 0.5D and 100% under 1.0D. In the 16 eyes implanted with a toric IOL, mean post-operative refractive cylinder was 0.26±0.38D. IWA suggested a different T power in 37.5% of cases when compared to the online calculator.
Conclusions:
IWA significantly outperformed the preoperative SBC IOL selection. This technology seems to have the potential to further improve accuracy in both spherical and toric IOL selection. Excellent refractive outcomes were obtained in this trifocal IOL platform.
