Purpose:
The commonly used method to compare the mean of the preoperative visual acuity with that of the postoperative visual acuity depends strongly on the composition of the study group with respect to severity of the disease. It does not consider the treatment potential which is different for mild and advanced cases. Comparing of different studies is also not possible. The purpose of this presentation is to propose a method to evaluate treatment success with respect to visual rehabilitation in keratoconus which considers the treatment potential of individual eyes and which allows comparison among different studies.

Methods:
Performance plot measures the line improvement by the treatment versus the preoperative visual acuity. Preoperative visual acuity is used as a measure of the severity of the disease. In the study 124 eyes suffering from mild to very advanced keratoconus treated by means of MyoRing implantation into a corneal pocket were analysed by means of performance plot technique. A linear regression analysis of the performance plot data was performed and compared to the hypothetical line for full visual rehabilitation to evaluate treatment success.
Results:
The improvement of uncorrected visual acuity was 7.97 +- 0.17 lines per logMAR. The improvement of corrected visual acuity was 8.13 +- 0.36 lines per logMAR. In case of full visual rehabilitation to visual acuity of 1.0 (20/20) postoperatively an improvement of 10 lines per logMAR is required. Performance plot analysis demonstrates some 80% success rate compared to full visual rehabilitation for both, corrected and uncorrected visual acuity.
Conclusions:
Performance Plot technique is an effective method to measure treatment success in visual rehabilitation of keratoconus and to compare treatment success among different studies. It may also be helpful as a tool during the training of surgeons in order to measure training progress or to compare different settings.
