Purpose:
To assess the agreement of the ocular aberrations measurements obtained with the Osiris pyramidal aberrometer (Costruzione Strumenti Oftalmici (CSO) S.r.l. Florence, Italy) and with the KR-1W Hartmann Shack aberrometer (Topcon, Tokyo, Japan) in healthy eyes.

Methods:
This prospective consecutive comparative case series study comprised 30 healthy eyes of 30 patients with age ranging from 21 to 50 years. Three consecutive measurements were obtained with both aberrometers. The high order aberrations (HOA) Root Mean Square (RMS) and all Zernike coefficients from the two order since the four order were analyzed. All aberrations were computed for 4.0mm diameter. Analysis of agreement and interchangeability was performed by the Bland and Altman method. In the intrasession repeatability by means of the intraclass correlation coefficient (ICC) was calculated.
Results:
Bland and Altman analysis showed good agreement between instruments only statistical significant differences between aberrometers were observed only for the HOA RMS, and Z(3 1) and (Z4 -4) coefficients (p≤0.02). For these parameters the ranges of agreement were 0.156, 0.402 and 0.056µm, respectively. The intrasession repeatability was similar with both aberrometers with high values of ICC. The ICC for the HOA RMS and Zernike coefficients obtained with KR-1w ranged from 0.733 to 0.999 and with the Osiris range from 0.628 to 0.999.
Conclusions:
The Osiris and KR-1W aberrometers provide an equivalent and repeatable measurements of ocular aberrations and can be used interchangeably for these purposes.
