Purpose:
Purpose: To investigate the Association of Meibomian gland dysfunction severity with Glycohemoglobin (HbA1c) level
Setting:

Methods:
In a cross-sectional, case-control study,160 eyes of 40 diabetic and 40 non-diabetic males and females with MGD were recruited. The Mean age of the patients was 56.54±8.45 and 104(65%) eyes belonged to female patients. An expert ophthalmologist determined the MGD stage considering the standard staging scale(American-Academy-Of-Ophthalmology). The diabetic state was evaluated according to the American Diabetes Association guidelines.The HbA1c level was measured in both groups applying a standard method, certified by the National Glycohemoglobin Standardization Program(NGSP). Patients with history of any refractive surgery, thyroid eye disease, regular consumption of antihistamines or tricyclic- antidepressants medications and Pterygium were excluded.
Results:
In this study 31.3% of the individuals had a minimally-altered MGD-stage, 46.3% were altered mildly and 22.6% of were altered moderately-severely. We observed that HbA1C levels significantly differed in each MGD-stage(p<0.0001). More precisely, different categories of HbA1c(<6.5%) were significantly associated with MGD stages(p<0.0001). So that, moderate-severe MGD-stage was observed in 77.8% of individuals with HbA1C≥ 6.5%, while it was observed as 43.2% in HbA1C 5.7-6.5% and 8% in HbA1c<5.6%. This association was not confounded by age or gender(Mutlinomial- logistic-regression). Moreover, with each year increase in age, the risk of Moderately-severely MGD was significantly increased(OR=1.19(95% CI),1.04-1.35;p=0.008).
Conclusions:
Meibomian gland dysfunction severity has a strong association with Glycohemoglobin (HbA1c) level and a rise in HbA1C aggravates MGD stage noticeably, so that along with one percent increase in HbA1c-level, the risk of Moderate-severe MGD increases remarkably (OR=2.87(95%CI), 1.55- 5.32; p=0.001).
