Abstract Details

Purpose:

Corneal transplantation had been done in UKMMC since 2008. This research was intended to document the surgical outcome and the incidence of complication of corneal transplantation surgery at 3, 6 and 12 months post-operatively in UKMMC.

Setting:

Ophthalmology Department, Universiti Kebangsaan Malaysia Medical Centre.

Methods:

This is a retrospective cohort study.Data were collected from the Cornea transplant registry at UKMMC and patients’ medical records. Patients selected had underwent corneal transplant surgeries from 2008 until 2013. Pre-operative comorbid conditions, post-operative visual acuity (VA), graft survival and complications were analysed at 3, 6 and 12 months.A total of 111 post-keratoplasty eyes [72 post-Penetrating Keratoplasty (PK), 26 post-Descemet’s Stripping Endothelial Keratoplasty (DSEK) and 13 post-Deep Anterior Lamellar Keratoplasty (DALK) eyes] were analysed.

Results:

The most common ocular comorbidity was glaucoma (22.5% n=25). Graft survival at 12 months was highest for post-DALK eyes (92.3%, n=12) followed by post-PK (75.0%, n=54) and post-DSEK eyes (57.7%, n=15). Post-PK and post-DSEK eyes showed statistically significant improved visual outcome after 12 months. Post-operative 1-year VA was best recorded in post-DALK eyes (logMAR=0.39) followed by post-PK (logMAR=1.00) and post-DSEK eyes (logMAR=1.15), p<0.05. The highest rate of complication encountered after the surgery was ocular hypertension (24.3%, n=41), suture-related problem or suture abscess (21.9%, n=37), cornea vascularization (20.7%, n=35) and graft failure (13.6%, n=23).

Conclusions:

Post-DALK eyes had the highest graft survival at 1 year. Most eyes showed excellent 1-year visual outcome following corneal transplantation surgeries in UKMMC. Ocular hypertension was the most common post-operative complication.
