Purpose:
To evaluate the functional outcome and patient’s satisfaction after bilateral implantation of Refractive Extended Depth of Focus Intra Ocular Lenses (IOLs) in myopic patients (MiniWell - Sifi S.p.A - Catania, Italy).

Methods:
We implanted MiniWell IOLs in 30 eyes (15 patients) with high myopia (>6.5D) and without any underlying ocular disease. IOL power was calculated using the SRK-T formula with Lenstar Biometer, chosing IOL powers able to achieve emmetropia. Patients were examined 1 months after surgery with uncorrected visual acuity (UCVA) and best distance corrected visual acuity (BDCVA) at 4 m, 40 cm, 66 cm and 80 cm together with defocus curve, aberrations with Hartmann-Shack aberrometer, quality of vision with OQAS, corneal tomography with Scheimpflug camera, quality of life with NEI-RQL-42 score test.
Results:
All enrolled patients attended the follow-up visits. After 1 month from surgery, patients implanted with MiniWell IOLs obtained a mean BDCVA and UCVA of respectively -0.05 and -0.05 LogMAR at 4m, -0.03 and 0.00 LogMAR at 80cm, 0.00 and 0.03 LogMAR at 66 cm, 0.15 and 0.08 LogMAR at 40cm. Mean ocular RMS was 0.24µm, mean MTF Score was 23.71. NEI-RLQ-42 questionnaire scores were 87.5 for dependence on correction scale, 96.4 for symptoms scale and 75 for glare scale.
Conclusions:
IOLs EDOF are considered a suboptimal choice for myopic eyes due to difficulties in IOL calculation, effective lens position (ELP) forecasting and the presence of high order aberrations. All patients got a satisfactory visual acuity and spectacle independence with reduced glare and other symptoms. RMS and MTF values suggest low aberrations with good contrast sensitivity. Defocus analysis showed that IOL positioning in the antero-posterior axis do not influence significantly UCVA. MiniWell IOLs seem to be a valid option for myopic patients regarding functional outcome and patients’ satisfaction.
