Comparison of optical performance and patient satisfaction with an extended-range-of-vision and a trifocal intraocular lens: a randomised prospective study

Purpose:
To compare the visual outcomes and subjective visual quality between bilateral implantation of a diffractive trifocal lens and an extended range of vision (ERV) intraocular lens (IOL).
Setting:
Hospital of St. John of God, Vienna, Austria.
Methods:
This prospective study included 40 eyes of 20 patients in the Symfony IOL group (=ERV group; Johnson & Johnson Vision Surgical, Inc. Santa Ana CA) and 34 eyes of 17 patients in the AT LISA tri IOL group (=TRIF group; Carl Zeiss Meditec, Jena, Germany). Follow-up examinations were performed 4-8 weeks and 6 months postoperatively and included: monocular and binocular uncorrected (UDVA) and corrected distance visual acuity (CDVA), binocular uncorrected intermediate (UIVA) and near visual acuity (UNVA), refraction, binocular defocus curve, photopic and mesopic contrast sensitivity, and responses to a surgeon and patient questionnaire.
Results:
Six months postoperatively, mean values in the ERV and TRIF group, respectively, were SE -0.07±0.21D and 0.02±0.28D; binocular UDVA -0.08±0.08logMar and -0.05±0.05logMar; UIVA -0.04±0.09logMar and -0.03±0.08logMar; UNVA -0.08±0.08logMar and -0.05±0.10logMar with no significant differences between both groups. The defocus curve showed significantly better outcomes for ERV IOLs at defocus levels of -0.50D and -1.00D, and for TRIF IOLs from -2.50D to -3.50D (p<0.05). Contrast sensitivity was comparable between the groups. In the TRIF group, 47% of patients reported mild perception of halos (versus 21% in the ERV group).
Conclusions:
The Tecnis Symfony ERV and AT LISA tri IOLs showed comparable visual performance at distance, intermediate and near. Predictability of the refractive results and optical performance were excellent. The two IOLs provided similar high contrast sensitivity in photopic and mesopic conditions. In the trifocal IOL group, a higher percentage of patients reported the perception of postoperative halos.
