Purpose:
To compare the ocular optical quality and subjective quality of vision after cataract surgery with bilateral implantation of a trifocal diffractive or an extended range of vision (ERV) intraocular lens (IOL).

Eye Clinic University of Verona, Verona Italy; Carones Ophthalmology Center,
Milan, Italy.
Methods:
This prospective study comprised 80 eyes of 40 patients undergoing cataract
surgery with bilateral implantation of the ERV IOL Tecnis Symfony (44 eyes, 22 patients; Johnson & Johnson Vision Surgical, Inc. Santa Ana CA) or the trifocal IOL Acrysof PanOptix (36 eyes, 18 patients; Alcon, Fort Worth, TX, USA). Included were patients with significant bilateral cataract, seeking for spectacle independence and with a pre-existing corneal astigmatism of less than 1.00 D. Three months postoperatively, ocular optical quality (OQAS system, Visiometrics), subjective quality of vision and patient satisfaction outcomes (NEI RQL-42 questionnaire) were analyzed.
Results:
Mean Strehl ratio and cutoff frequency of modulation transfer function (MTFcutoff) were slightly higher in the ERV group, but differences between IOLs did not reach statistical significance (p≥0.058). Patient questionnaires showed no significant differences between IOLs (p≥0.180), except for clarity of vision, with higher values obtained in the trifocal group (p=0.016). Mean score for distance vision was higher in the ERV group (ERV: 87.65±13.18; trifocal: 84.50±14.90). For near vision, the trifocal group showed somewhat better results (trifocal: 90.81±16.00; ERV: 86.11±15.89). Patients reported a mean satisfaction score of 87.27±21.86 (trifocal) and 90.00±14.14 (ERV).
Conclusions:
Both the ERV and the trifocal IOL provided comparable outcomes regarding optical quality and subjective quality of vision 3 months after implantation.
