Abstract Details

Purpose:

To evaluate the efficacy and safety of 320o ICRS segments in central and paracentral keratoconus.

Setting:

Menoufia university and Tiba Eye & Lasik Center, Shebin Elkom, Menoufia, Egypt

Methods:

A prospective study was performed on 32 eyes of 22 patients with keratoconus. All eyes were implanted with femtolaser assisted 320o ICRS segments. 24 eyes implanted with 320/300 segments and 8 eyes implanted with 320/200 segments. Preoperative evaluation included determination of uncorrected (UCVA) and best corrected visual acuity(BCVA). Also, potential visual acuity (acuity with RGB contact lens on). Manifest and cycloplegic refraction were evaluated and recorded with decimal fraction. Pentacam examination was carried out with Allegro Oculyzer machine. Conventional collagen cross linking (CXL) was carried out at 8 weeks postoperative in all cases.

Results:

At 6 months after implantation of 320o ICRS the preoperative UCVA has improved from 0.05 ± 0.12 to 0.20 ± 0.05 and the BCVA has improved from 0.15 ± 0.06 to 0.40 ± 0.08. The cornea has become flatter. The mean preoperative K1 has changed from 54.54 ± 2.31 to 48.42 ± 2.21 diopters (D) and the mean K2 has changed from 58.64 ± 2.62 to 51.25 ± 2.54 diopters. The manifest preoperative refraction has changed from -8.5 ± -1.35 to -2.11 ± -1.22 D. One case showed delayed epithelial healing for 17 days.

Conclusions:

320o ICRS were safe and effective in the management of central and paracentral keratoconus. It was safely adjuncted with collagen cross linking without significant complications.
