Long-term outcomes of keratoconus management with corneal wavefront-guided normalisation combined with corneal cross-linking stabilisation

Abstract Details
Purpose:
To investigate refractive, topometric, pachymetric, and visual rehabilitation changes induced by anterior surface normalization for keratoconus by corneal wavefront-guided (CW) excimer laser ablation in conjunction with corneal cross-linking (CXL).
Setting:
Veni-Vidi Eye Hospital, Istanbul, Turkey
Methods:
Forty eyes of 25 keratoconic cases subjected to the procedure were studied for visual acuity, keratometry, pachymetry, and anterior surface irregularity indices by wavefront analysis system.
Results:
The mean age of the patients was 32.22 years (20 to 56 years) and the mean follow-up was 36.5 ± 6.5 months (6 to 48 months). Pre-operatively, all eyes had irregular astigmatism ranging from 0.25 to 6.00 diopters (D). The means of pre- and post- operative RMS of high order aberrations were 2.28 ± 1.16 and 1.24 ± 0.76 (p=0.005). The means of pre- and post- operative uncorrected and corrected visual acuities were 0.18±0.08, 0.28 ±0.2 and 0.42 ±0.26, 0.80±0.19 Snellen lines respectively (p1=0.08, p2=0.001). 22 eyes (55%) had an increase of more than 5 lines.
Conclusions:
Long term outcomes have demonstrated that the procedure is a safe and effective method to arrest keratectasia progression and to improve corneal regularity in keratoconus management.
