Changing our minds about predisposing factors for post-LASIK ectasia

Abstract Details

Purpose:

This study aims at studying unexpected predisposing factors of post-Lasik ectasia.

Setting:

Elnour Eye Centre for Lasik, Asyut.

Methods:

This retrospective non comparative case series study included 14 eyes in 7 patients underwent LASIK at El Nour Eye Centre for Lasik in duration of 3 years (from March 2015 till November 2018), during follow up, we found these patients had developed ectasia in the duration of 12-24 months after Lasik procedure, repeated work up were done including: pentacam, refraction evaluation proved that they have ectatic corneas.

Results:

We had equal incidence (28.5%) of post-lasik ectasia cases in age groups (18-20y), (20-25y) and (30-35y).43% of cases had high myopia (6-10D), the same incidence for patients who had moderate myopia (>10D).29% of cases had pachymetry of (480-500µm) while 71% of cases had (500-520µm).43% of cases had K max of (42-44D), the same incidence for those had K max of (44-46D).In Belin Amb'rsio Enhanced ectasia display, (100%) of cases had normal difference map; while 72% had suspicious exclusion map, and 14% had risky exclusion map; "D"score was suspicious in about 42% of cases and risky in 7%.

Conclusions:

This study has changed mind about post-Lasik ectasia predisposing factors; all age groups are in equal risk of developing ectasia after Lasik procedure. Pre-operative mild & moderate myopia, corneal pachymetry >500µm and flat corneas are not guaranteed against developing post-Lasik ectasia. Exclusion map and "D" score in Belin Amb'rsio Enhanced ectasia display are very important in detecting the possibility of ectasia occurrence.
