Discussion of diagnosis and management of special scenario of decrease of vision in post-LASIK eyes

Abstract Details

Purpose:

We describe the management of a unique case of reappearance of myopia after Laser assisted in situ keratomileusis(LASIK) due to pressure induced interlamellar keratitis(PISK)

Setting:

Advanced eye centre,Chandigarh,India

Methods:

PISK is diagnosed when patient presents with blurring of vision with raised intraocular pressure and on slit lamp biomicroscopy interface haziness is seen and it is confirmed on anterior segment optical coherence tomography(ASOCT)

Results:

Rapid resolution of stromal haziness and disappearance of lakes of fluid in the interface occurs on discontiuing steroidal medications and on starting antiglaucoma drugs.

Conclusions:

PISK presents with clinicial features similar to diffuse lamellar keratitis(DLK) but its onset is typically after 1 week of surgery and responds dramatically to pressure lowering drugs and stopping of steroids. It is essential to monitor patients post LASIK for their pressures, thorough vision documentation and  slit lamp examination.ASOCT provides a useful tool for monitoring patients with interface haziness and fluid after refractive procedures.
