Efficiency analysis of intrastromal corneal segment (ICR) implantation among patients with keratoconus

Abstract Details

Purpose:

Analysis of ICR implantation using femtosecond laser results in case of keratoconus.

Setting:

St. Petersburg branch of S. Fyodorov Eye Microsurgery Federal State Institution, 21 Yaroslav Gasheka str., St. Petersburg, Russian Federation, 192283

Methods:

38 patients were observed (54 eyes), with stage 2-3 of keratoconus according to M. Amsler classification. 7 women and 31 men were among them aged 18-62 (35.26 ± 11.95 years). All were subjected to a standard ophthalmologic examination, including Pentacam® (OCULUS Optikgeräte GmbH, Germany) and OCT (RTVue 100, Optovue, USA). Evaluated: tunnel formation depth, uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), spherical equivalent (SE) and the cylindrical component of refraction, as well as the maximum of cornea refraction on keratotopograph before the operation, on the 1st day after operation, in 3, 6 and 12 months after ICR implantation.

Results:

UCVA preop from0.02to0.8(0.18 ± 0.18),BCVA preop - from 0.3to1.0(0.69 ± 0.21). SE - from -9.0 to 3.0 D (-1.38 ± 3.23 D), the cylindrical component - from -15 to 4.0 D (-5.04 ± 3.37 D). first day: UCVA from 0.02 to 1.0 (0.35 ± 0.27), BCVA from 0.1to1.0 (0.76 ± 0.23), SE - from -9.0 to 2.0 D (-1.0±2.59 D), cylindrical component - from –10.0to2.0 D (-2.63 ± 2.92 D). 1-month results: UCVA averaged 0.73±0.33, BCVA 0.81 ± 0.24, SE –0.75 ± 2.46 D, cylindrical component -1.85±2.88 D. In 3 and 6 months, all indicators are stable.

Conclusions:

ICR implantation using femtosecond laser in treatment of keratoconus is a safe technique. Implantation of ICR with corneal tunnel formation using VisuMAX femtosecond laser in patients with keratoconus is an efficient correction method for refractive disorders that go with this disease.Optical cornea reshaping and associated reduction of spherical and cylindrical refraction component during ICR implantation significantly increases the visual acuity with /without correction and improves the quality of life among patients with keratoconus. .
