Unilateral corneal ectasia post-KAMRA insertion

Abstract Details
Purpose:
To report on two cases of unilateral corneal ectasia which developed after KAMRA (AcuFocus Inc., Irvine, CA, USA) inlay insertion. In both patients, corneal ectasia was diagnosed 5 years after the initial procedure. Routine preoperative screening was within normal limits with no signs of forme fruste keratoconus. To the best of our knowledge, these are the first 2 cases in the literature of corneal ectasia following KAMRA inlay insertion.

Single Refractive Centre in Toronto, Canada
Methods:
A retrospective chart review was performed on both cases. Demographics including age, sex, laterality and refractive and corneal tomography status were collected. Outcome measures were post-operative refraction and corneal tomography and topography measurements and best corrected visual acuity. Eye rubbing history was elucidated.
Results:
Pre-operative evaluations for both cases demonstrated showed healthy corneas with no evidence of forme fruste keratoconus. Five years after the procedure the patients presented with a subjective decrease in visual acuity. Topographic and tomographic maps were consistent with corneal ectasia. A significant myopic shift was noted in one case and a significant hyperopic shift was noted in the other.
Conclusions:
Corneal ectasia is a long-term risk for patients with KAMRA inlay insertion. These are the first cases of corneal ectasia reported in the literature following Karma inlay insertion. Careful preoperative evaluation and continued follow up is paramount in preventing the presentation and ensuring early detection of this condition to prevent complications.
