Thermal damage of corneal inlay due to cosmetic eyelid procedure

Abstract Details
Purpose:
To present a case of explantation of KAMRA inlay (AcuFocus, Inc., Irvine, CA) due to decreased vision caused by thermal damage of inlay after cosmetic treatment of eyelid skin with long pulsed 1064-nm Nd:YAG laser.
Setting:
Public Eye Hospital
Methods:
Case report of a patient referred by a private aesthetic surgeon. Patient underwent myopic LASIK and KAMRA inlay implantation 5 years before this incident.
Results:
Patient reports decreased vision within 30 minutes of completing laser treatment over eyelids for facial skin tightening. Clinically significant corneal haze associated with extensive KAMRA inlay thermal damage was observed. Patient required corneal flap lift, washout and removal of the inlay.Three months following explantation, patient regained corrected distance vision but residual corneal haze remains. Histopathology of the explanted corneal tissue-inlay complex shows thermal injury and disintegration of the inlay.
Conclusions:
Healthcare professionals need to be aware of this complication when performing cosmetic laser treatment in the periorbital region of patients with KAMRA inlays. Avoiding the periorbital region and/or the use of a corneal shield is recommended when performing cosmetic laser treatments in patients with KAMRA inlays.
