Clinical findings in the postoperative course of cataract surgery with toric intraocular lens (IOL): is macular optical coherence tomography (OCT) necessary as a routine preoperative test?

Abstract Details

Purpose:

To describe the prevalence of incidental findings in the posterior segment in a sample size of 30 patients who underwent cataract surgery with toric IOL implantation.

Setting:

Department of Ophtalmology Hospital de l`Esperança – Parc de Salut Mar Barcelona-Spain

Methods:

We present an observational descriptive study of a sample size of 30 patients who underwent phacoemulsification with toric IOL implantation. All the surgeries were performed by the same surgeon; in a range between 23 months, since march of 2017 and january of 2019. The prevalence of incidental findings in the posterior segment found after the surgery was reported in a table that describes the sample size of patients, demographic data and codifications for each casual finding in the posterior segment of the eye.

Results:

We report a prevalence of 23,3% (7/30) incidental findings after the phacoemulsification with toric IOL implantation in this sample of patients. 5 patients had epiretinal membranes; one of which had associated a macular vitreous traction; and 2 patients had age-related macular degeneration. Compared with others studies where a percentage up to 12.9% of posterior segments findings were found after cataract surgeries.

Conclusions:

Surgical complications and incidental postoperatory findings in the posterior segment are causes of uncorrected distance visual acuity (UCDVA) or distance spectacle independence not gained as expected. We propose the use of complementary tests in the preoperative evaluation such as macular OCT in addition to the slit lamp exploration according to the individual clinical characteristics.
