Clinical case of refractive lens exchange in patient with occult choroidal neovascularisation

Abstract Details

Purpose:

To submit a clinical case of phased treatment of a patient with a high degree of hyperopia in the presence of an inactive occult choroidal neovascularization (CNV).

Setting:

1 ophthalmology center “Vzglyad” 2 SI “Zaporizhzhia Medical Academy of Post-Graduate Education Ministry of Health of Ukraine”

Methods:

Presented clinical case of a 51-year-old man with high degree hyperopia, refractive lens exchange and implantation of a multifocal intraocular lens (IOL) Panoptix. OСT was perform at the preoperative stage. A small local detachment of the pigment epithelium was find at the one eye. OCT-angiography confirmed the presence of CNV without sign of activity. Surgery was delay for dynamic observation and treatment. Six months later, taking into account the stable local detachment of the pigment epithelium, a refractive exchange of the lens was perform for both eyes with trifocal IOL implantation. Visual acuity was 1.0 on the second day.

Results:

5 days after surgery, the patient noticed the blurred vision at the eye with CNV. Visual acuity was 0.5.  Control OСT: the signs of membrane activation were detected (edema in the center of retina and detachment of the neuroepithelium). Anti-VEGF therapy was performed.  After that completed adherence of neuroepithelium detachment, fluid resorbtion, decrease in neovascularization activity were notice. Visual acuity returned to 1.0.  Patient continued intravitreal aflibercept on the “ treat and extend”   scheme.

Conclusions:

All patients for cataract surgery at the preoperative stage should undergo ОСT control of the central retinal zone for definition the prognosis, tactics and volume of treatment. In case of an inactive occult CNV detection the refractive replacement of the lens is not contraindicated, but it implies a step-by-step treatment according to the standard scheme.
