Comparison of the refractive and reported functional outcomes after implantation of the TECNIS Symfony, an extended-range-of-vision intraocular lens (IOL) in post-refractive and non-refractive patients undergoing cataract extraction

Abstract Details

Purpose:

To analyse the clinical outcomes of cataract patients after implantation of an extended range of vision intraocular lens (IOL), the Tecnis Symfony, in post refractive and non-refractive patients in a routine clinical setting

Setting:

Foulkes Vision Institute clinic in Lombard, Illinois

Methods:

Retrospective study on consecutive cataract operations performed on 39 patients (mean age 61.8 years) from May 2017 to July 2018.  Refraction, uncorrected distance (UDVA) and uncorrected near visual acuity (UNVA) were evaluated preoperatively and postoperatively on 1 week-1 month and 3-6 months follow up. Patient satisfaction, spectacle independence defined as never wearing glasses and dysphotopsia defined as noticeable visual disturbance though not visually significant were evaluated using a survey.

Results:

Postoperative evaluations were completed by 39 patients (67 eyes). 28 patients had bilateral IOL implants whereas 11 had unilateral IOL implant. 3-6months postoperative UDVA 0.18 LogMAR (20/30) or better was achieved in 94.9% (37/39) patients (94.7% post-refractive, 90% non-refractive).  76.9% (30/39) patients had UNVA 0.18 LogMAR (20/30) or better (78.9% post-refractive, 75% non-refractive). Spectacle dependence for reading were higher in non-refractive (66.67%) compared to refractive patients (33.3%). 72.7% patients reported some dysphotopsia. Mean patient satisfaction was 7.2 as some patients expected better near and far vision. 18 patients achieved a refractive goal of -0.50- +0.50 of spherical equivalent refraction.

Conclusions:

Both refractive and non-refractive patients had better UDVA than UNVA 3-6months post IOL implant. Post refractive patients were less spectacle dependent for reading. Overall, patients reported better near compared to far vision after 6 months. Post refractive did not associate with higher visual aberration.  The Symfony implant placed before correction with Toric IOL availability provided excellent patient satisfaction and reduced need for glasses. The overall result may have been better with zero or less astigmatism. The Post refractive use appears to be effective in this small sample.
