Comparison of three diffractive intraocular lenses after cataract surgery

Abstract Details

Purpose:

To compare the visual outcomes of three diffractive intraocular lenses (IOL’s): a panfocal model, a trifocal model and a low-power bifocal model.

Setting:

An independent eye clinic in the UK.

Methods:

A prospective comparative case series of patients undergoing cataract surgery who had bilateral implantation of one of the three following IOL’s: Panoptix (Alcon), AT lisa trifocal (Zeiss) and the Symfony (J and J). Emmetropia was targeted for both eyes with the Panoptix and AT Lisa. The Symfony patients were set-up with myopia of -0.5 to -1 in the non-dominant eye. Post-operative examinations included visual acuity measurement at 4m and the patients preferred distance for near vision (30 to 40cm) and intermediate vision (60cm to 80cm). A questionnaire was administered to assess spectacle independence and quality of vision.

Results:

The study comprised 30 patients in each group. There was no statistically significant difference between groups for mean post-operative binocular UCDVA (Panoptix -0.05 +/- 0.08 logMAR, AT Lisa -0.08 +/- 0.06, logMAR, Symfony -0.11 +/- 0.08 logMAR) or mean binocular UCIVA at the patients’ optimum distance (Panoptix 0.39 +/- 0.08 logMAR, AT Lisa 0.4 +/- 0.09 logMAR, Symfony 0.43 +/- 0.1 logMAR). The monocular UCNVA was statistically significantly better in both the Panoptix (0.19 +/- 0.12) and AT Lisa (0.18 +/- 0.1) groups compared to the Symfony group.0.33 +/- 0.14. There was no difference in binocular UCNVA between groups however.

Conclusions:

Using the weak bifocal IOL in a mini-monovision set-up allowed equivalent unaided near vision compared to the Panoptix and AT lisa IOL’s. A larger proportion of patients in the Panoptix group achieved complete spectacle independence than in the AT lisa and Symfony groups but this was not significant. There was no difference in the frequency or severity of glare and halo symptoms between groups.
