Comparative analysis of implantation behaviour of two preloaded IOL systems and its effect on visual function

Abstract Details

Purpose:

To evaluate the Implantation Behavior of Piston type preloaded Intra Ocular Lens Systems and Screw Type preloaded Intra Ocular Lens Systems and its effect on Visual Function

Setting:

This is non randomized and comparative study which included 100 eyes of 50 patients at a Tertiary Eye Care Hospital, India

Methods:

This was a non randomized and comparative study of 100 eyes in 50 patients who were divided into two groups i.e. Screw type Preloaded Intra Ocular Lens system (Gr-1) and Piston Type Preloaded Intra Ocular Lens system (Gr-2) at a tertiary care hospital. All surgeries were performed by a single experienced surgeon for approximately the same operating time. Any variance was removed from the list. Per operatively IOL Implantation behavior (haptic Extension) and Wound stretch were monitored and its effect on Post-Op Visual Acuity (VA) at 7 days and 90 days were evaluated

Results:

Haptic Extension was seen in 28 % in Screw Type IOL group while no case was reported with Extension in the Piston Type IOL group. The difference is found to be significant p=0.0001 Pre operative Change in width is significantly more in Gr-1 as compared to Gr-2, difference calculated is  22.700, P = 0.000, 95% CL(0.4326 to 0.5154) Mean of VA in Gr-1 had comparable results with Gr 2 at 7 days (0.99, 0.97, p=0.205, 95%CI(-.00809 to .03729) VA decrease not significant in Gr-2 (mean= 0.93) as compared to Gr 1 (mean=0.97) at 90 days, p=0.051, 95%CI (-0.00020 to 0.07700).

Conclusions:

In our study we found that Piston IOL System has more control during delivery resulting in significantly lesser wound stretch and maintains wound integrity with the incision guard. The incidence of haptic extension was found to be significantly more in Piston Type Systems. The change in Visual Acuity is comparable at 7 days and 90 days and the difference is statistically insignificant (p<0.000).
