Management of a residual refractive error after complicated LASIK surgery and irregular astigmatism

Abstract Details

Purpose:

Case report: To show how we can best manage a residual refractive error after complicated LASIK surgery and irregular astigmatism.

Setting:

A 58-year-old patient proceeded to myopic LASIK(-8D) by the age of 51. She underwent a retreatment in her OD because a refractive error occurred after the first procedure.

Methods:

In OD, was UCVA=1.00logMA. The refractive error was +0,75/-0,50/A172° and the Biometric-data was K1=37.01D and K2=38,52D, Pachymetry 441µm, AL=26,39mm, ACD=3.32mm, WTW=12,5mm. She also had in OCT an epiretinal membrane but no macular edema. proceeded to the femtosecond-assisted phacoemulsification and implantation of an IC-8 IOL AcuFocus, because our patient wanted a spectacle-free vision, if possible.

Results:

No complications occurred during surgery, and two months after the surgery the UCVA OD was 0,40logMAR, and BCVA was 0,20logMAR with a refractive error of -1.25/-1,65cylA175°. In OCT, the epiretinal membrane was stable with no macular edema.

Conclusions:

Calculating the correct lens power in patients after LASIK is a known difficulty, and it is even more difficult to determine the correct lens for each patient. For this patient, the choice of a monofocal IC-8 lens was the best selected IOL with the best visual outcomes, even if the refractive error was not completely eliminated. Our patient is now satisfied.
