Causative analysis of explanted iris-fixated phakic intraocular IOLs

Purpose:
To report the main causes leading to explantation of iris-fixated phakic intraocular lenses (pIOLs).
Setting:
The David J. Apple International Laboratory for Ocular Pathology
Methods:
In this retrospective study, 30 eyes (26 patients) that underwent pIOL explantation due to various causes were studied. Following pIOL models were included: Artisan (Ophtec GmbH, Groningen, Netherlands), Artiflex (Ophtec GmbH, Groningen, Netherlands), Verisyse (Johnson & Johnson, Santa Ana, USA), and MicroSil Iris-Fixated Toric (HumanOptics, Erlangen, Germany). Light microscopy images were recorded and the patients’ clinical data analyzed.
Results:
The mean age of the patients at the date of explantation was 56.3 ± 12.4 years (range: 25 to 76 years). The mean time between the dates implantation and explantation was 11.0 ± 6.7 years (range: 0.3 to 20.4 years). The primary causes leading to pIOL explantation were endothelial cell loss (36.7%, 11 cases), cataract development (30.0%, 9 cases), and pIOL dislocation/decentration (16,7%, 5 cases). In three cases (10%), the pIOLs had to be explanted due to severe inflammatory reaction in the anterior segment. Other causes included intraoperative complications and secondary explantation due to corneal graft failure.
Conclusions:
We found that endothelial cell loss was the most common cause of explantation in pIOLs. The second most common cause was cataract formation.
