Management of high astigmatism in keratoconus and post-keratoplasty with phakic intraocular lenses and its outcome

Abstract Details

Purpose:

To compare the safety and efficacy of phakic lens in correcting ametropia following collagen cross-linking (CXL) in keratoconus and post-keratoplasty cases and secondarily to compare the visual outcomes of phakic IOL implantation in post-CXL vs post-keratoplasty cases.

Setting:

sankara eye hospital coimbatore India.

Methods:

This is a prospective study of patients post-collagen cross-linking and post-keratoplasty who were implanted with phakic intraocular lens implantation (pIOL). The main outcomes measured were uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), spherical and cylinder refraction.

Results:

This study included 56 eyes (35-post CXL, 21 post keratoplasty) . Overall, The UDVA and CDVA improved 1.12 to 0.31 logMAR (p <0.01), and improvement in CDVA from 0.31 to 0.2 logMAR (p <0.05) respectively.In the 35 post- CXL the refraction improved from -6.82 ± 4.1D and -0.14 ± 0.29D to -4.1 ± 1.58D and -0.8 ± 1.29 respectively. Similarly, in the 21 post–keratoplasty cases the UDVA improved from 1.12 to 0.38 logMAR (p<0.01), while CDVA improved from 0.38 to 0.22 logMAR (p< 0.01).

Conclusions:

PHAKIC IOL is an effective and safe method of improving visual acuity and refraction in keratoconus and post-keratoplasty eyes with high myopia and astigmatism.
