Role of dynamic measurements of vaulting in a possible ICL exchange

Abstract Details

Purpose:

To assess the decision making process involved in two cases of ICL© exchange with low vault based on dynamic measurements of vaulting.

Setting:

Clínica Baviera Madrid, Spain

Methods:

Using anterior segment optical coherence tomography (CASIA SS-1000; Tomey), we dynamically evaluate the vault of two phakic collamer intraocular lens (ICL© EVO+ model) under variable ambient light conditions. Through photographs and video captures, the following parameters were measured: vault interval (VI), defined based on central vault values in maximum mydriasis and miosis after light-induced changes in pupil diameter, vault range (VR), defined as the absolute difference between the VI values; and the crystalline lens rise (CLR), defined as the distance between the anterior pole of the crystalline lens and the horizontal line that joins the two irido-corneal angles.

Results:

In the first case, VI values of the ICL© (13.2/-9 diopters) were 63 and 16 microns in mydriasis and miosis, resulting the VR in 47 microns. CLR values were 481 and 586 microns, in mydriasis and miosis respectively. Due to the proximity of the ICL© to the crystalline lens in miosis, it was decided to perform an exchange by a higher size (13.7). In the second ICL© (12.6/-8 diopters), VI values were 80 microns in mydriasis and miosis, resulting in a VR unchangeable, remaining a stable CLR of 100 microns. An expectant attitude was assumed, and future follow-ups were scheduled.

Conclusions:

New dynamic vault parameters describe more precisely the ICL© behavior. These parameters should be incorporated routinely in the evaluation of postoperative pIOL vaulting.
