Changes in endothelial cell count up to three years after implantation of the toric implantable collamer lens (TICL)

Abstract Details
Purpose:
To determine the temporal effect of toric implantable collamer lens(TICL) implantation and location on the corneal endothelial cell density over a period of 36 months following surgery.
Setting:
Specialty Eye Hospital Svjetlost, School of Medicine University of Rijeka, Zagreb, Croatia
Methods:
Endothelial cell density (ECD,number of cells per mm2 estimated using Specular Microscope SP-1P (Topcon Europe Medical B.V., Netherlands) data were harvested from cases deemed suitable for TICL(VTICMO,VTICM5,Staar Surgical,Nidau,Switzeland). The pre-op refractive error (sphere and cylinder) ranged from -1.00 diopter sphere(DS) to -22.25DS &-0.50 diopter cylinder(DC) to -5.50DC. ECD was evaluated at preop and all postop sessions.
Results:
Reporting the key findings. Mean ECD(±sd,95% CI ) at preop was 2720 cells/mm2(±272, 2620-2820) reducing to 2372 cells/mm2 (±325, 2250-2490) at 36 months postop (p<0.001). Linear regression revealed the following significant correlations between
A) Log of change in ECD (y1) and log of preop ECD (x1) at 2 years post,
y1 = 2.513x1-6.2816, (n=62,r= 0.3503,p=0.005).
B) Mean ECD (y2) and log time (in months,x2),
y2= 2543.7-36.997x2-38.99x22 (r=-0.9654, n=7,p=0.0004)
C) Mean axial distance between front surface of crystalline lens and TICL back surface (y3) and time postop (in months, x3),
y3 = 0.1035x32-5.2808x3 +473.18 (r=0.8512,n=7, p=0.015).
Conclusions:
The expected ECD loss after TICL implantation by two years postop is predictable. On average, over three years after implantation there is i) an initial rapid decline in ECD followed by gradual fall in the rate of cell loss and ii) a gradual fall in the distance between the TICL and crystalline lens by two years postop followed by a reversal by the third year.
