The visual outcome of phakic iris-claw lenses in highly myopic eyes

Abstract Details
Purpose:
To report a favorable outcome of the insertion of Artisan phakic IOLs for the correction of high myopia.
Setting:
Al-Hussaini Hospital, Assiut, Egypt.
Methods:
This is a retrospective study on 33 eyes of 24 patients who were implanted with Artisan phakic iris claw lenses for the correction of high myopia. The decision to implant a phakic IOL was based on the finding of a high error, a thin cornea or both. Preoperative best-corrected visual acuity was compared postoperative uncorrected visual acuity. Selective suture removal was used to reduce or eliminate postoperative with the rule astigmatism. Complications were recorded.
Results:
The preoperative spherical equivalent refraction of the operated eyes ranged from -6 and -23 (average=-14.48). Twelve eyes (36.36%) achieved postoperative UCVA better than their preoperative BCVA. Sixteen eyes (48.48%) achieved UCVA equal to their preoperative BCVA. Five eyes (15%) had UCVA of one line less than their preoperative BCVA. A shallow AC and cornea edema happened in one eye and need reformation of the AC with additional suturing. Intraoperative hemorrhage occurred in one eye and was controlled by pressurizing with Helon and no effect on the outcome. One lens dislocated due to blunt trauma and had to be re fixated.
Conclusions:
Artisan phakic iris claw lenses are a safe and effective choice for the correction of myopia when corneal refractive surgery is not suitable. Complications are rare and can be easily managed.
