When your ICL goes wrong

Abstract Details

Purpose:

To present different cases that were treated using the phakic IOLs Visian ICL and demonstrate the intraoperative complications and difficulties encountered in different cases and how to overcome them.

Setting:

King Hussein Medical Center Laser refractive center Amman Jordan.

Methods:

4 patients who underwent correction of their high refractive errors with the implantable Collamer Lenses-Visian ICL showed intraoperative and postoperative difficulties with the ICL. intraoperative high vaulting, reversed lens, and capture of the iris were all encountered. we will demonstrate how to deal with such difficulties intra-operatively with best surgical outcome.

Results:

In the four patients the ICL had to be removed from the eye with a certain technique. In case of high vaulting the white to white was rechecked and smaller lens ordered. A special technique was used to evaluate a reversed ICL in the eye and was pulled out and re-injected with minimal manoeuvring. One patient showed an iris plateau and shuttering effect that caused iris capture over the ICL! This was managed by removal and replacement with trifocal IOL lens.

Conclusions:

Surgical experience as well as a good clinical eye are important for the ophthalmologist when performing premium IOL surgeries. Early recognition of a problem makes it easier to deal with and corrects it with minimal damage and minimal manoeuvres ensuring best refractive outcome.
